Mission Statement
The clinic's primary goal is to offer family physicians or other specialists an opportunity to obtain neurology consultation directly from Dr. Agnes Supala-Berger, MD, FRCPC. Our goal is to provide appointments in the timeliest manner.
However, in evaluating the benefits of a shared collaborative model in the hospital setting, we decided to adopt the possibility of providing a similar outpatient model by offering a comprehensive collaborative approach to neurological and related disorders.
The clinic is not a family practice. The clinic can provide dual care to patients with chronic neurological disease and rehabilitative needs along with the patient's family physician. The clinic offers episodic comprehensive assessments in the areas of prevention or disease management. The ideal method of accessing the clinic's services is to request a consultation to one or both of our physicians.
The clinic handles broad aspects of neurological diseases, for example a patient may want to be assessed to get a qualitative evaluation of risk factors for stroke and life style modification. Another patient may want to access the clinic for counseling on a neurological disease. A patient with epilepsy may want to take advantage of treatment to improve his or her management while at the same time adapt and maximize aspects of his or her quality of life. An athlete who has sustained a concussion or neuromuscular injury who wants to return to full activities may want to be assessed. A patient diagnosed with ALS or other progressive neurological disease may want to come to the clinic to get diagnosed or get a second opinion and at the same time receive support about advanced care planning.
Neurological illness often poses a multitude of challenges for patients and families that might benefit from a collaborative supportive approach in an outpatient clinic setting.
